Job Description and Requirements
Location: 2400 Belvidere Road, Waukegan IL (Belvidere Medical Bldg.)
Hours: Monday, Wednesday, Thursday, & Friday (8:00 am - 4:30 pm) Tuesday (10:30 am - 7:00 pm)
Summary: Under the supervision of the Nutrition Services Assistant Coordinator, develops and coordinates the Breastfeeding component of the WIC program. Coordinates the Breastfeeding Peer Counselor program, and supervises Breastfeeding Peer Counselors at all sites. Assists staff in the provision of direct nutrition services for WIC/Primary Care clients in accordance with program and agency requirements. Work involves direct client contact and is performed in accordance with state and local agency policies and procedures.
ÃÂ¢ÃÂ¢Coordinates and facilitates the Breastfeeding component of the WIC Program for all Lake County sites.
ÃÂ¢ÃÂ¢Coordinates and facilitates the Breastfeeding Peer Counselor program. Supervises Peer Counselors and evaluates/monitors Peer Counselor productivity and effectiveness. Develops, implements and monitors IDHS Peer Counselor grant.
ÃÂ¢ÃÂ¢Monitors breastfeeding rates and trends, determines needs and identifies strategies, policies and goals which will increase/support breastfeeding among WIC clients. Evaluates quarterly IDHS breastfeeding outcomes. Communicates with Nutrition Services Assistant Coordinator and the State Breastfeeding Coordinator regarding breastfeeding related issues and services.
ÃÂ¢ÃÂ¢Coordinates and provides breastfeeding education and other trainings for all WIC, Family Case Management, Primary Care Medical staff, and the community. Assists in the development and presentation of All Staff and Management Team trainings based upon programmatic and staff needs.
ÃÂ¢ÃÂ¢Oversees the breast pump program including ordering, monitoring, inventory and tracking of all breastfeeding pumps and breastfeeding supplies.
ÃÂ¢ÃÂ¢Assists Management team in overseeing staff and daily clinic activities of WIC services at largest clinic site to achieve caseload, client visit and program goals.
ÃÂ¢ÃÂ¢Provides certification, nutrition counseling, medical nutrition therapy (including Gestational Diabetes assessment and counseling), breastfeeding support, and appropriate referrals for WIC/Primary Care clients according to established criteria.
ÃÂ¢ÃÂ¢Performs quarterly Breastfeeding Competency Assessment observations for all Lead Dietitians and mentors Lead Dietitians to help improve breastfeeding promotion and support at their clinic sites.
ÃÂ¢ÃÂ¢Monitors and adjusts staff and client scheduling, including providing coverage for outlying sites.
ÃÂ¢ÃÂ¢Participates in ongoing program evaluation including Continuous Quality Improvement activities and monitoring of program/grant goals, objectives and performance measures.
ÃÂ¢ÃÂ¢Prepares reports on all staff activities, client data, and service delivery as required.
ÃÂ¢ÃÂ¢Revises, implements and monitors protocols, policies and procedures to ensure services meet program/grant objectives and are in compliance with Federal/State requirements.
ÃÂ¢ÃÂ¢Performs monthly competency assessments, quarterly/mid-year meetings, and annual performance appraisals for assigned staff and provides timely feedback in meeting established performance goals.
ÃÂ¢ÃÂ¢Monitors staff completion and accuracy of timecards.
ÃÂ¢ÃÂ¢Issues Medically Prescribed Formula according to protocol and monitors staff Medically Prescribed Formula issuance.
ÃÂ¢ÃÂ¢Assists in administrative functions related to food instruments and FarmersÃÂ¢ Market coupons.
ÃÂ¢ÃÂ¢Facilitates the integration of WIC services including breastfeeding promotion and support initiatives with other Community Health Center/Health Department programs and outside agencies.
ÃÂ¢ÃÂ¢Assists the Coordinator/Assistant Coordinator in the hiring, orientation and training of new staff and serves as a preceptor for dietetic interns and graduate students.
ÃÂ¢ÃÂ¢Provides program representation at internal and external meetings and committees.
ÃÂ¢ÃÂ¢Demonstrates support of service and program area philosophy and decisions in verbal and written communication with supervisors, staff, clients and community.
ÃÂ¢ÃÂ¢Participates in mandatory in-services and drills, attends staff trainings and other continuing education appropriate for professional development and job duties.
ÃÂ¢ÃÂ¢Develops and maintains a basic knowledge of services available at the LCHD/CHC.
ÃÂ¢ÃÂ¢Develops and maintains a basic awareness of the Chronic Care Model Processes in order to reduce Health Disparities within our communities.
ÃÂ¢ÃÂ¢Assists and encourages clients in identifying self-management goals.
ÃÂ¢ÃÂ¢Requires MasterÃÂ¢s Degree in Nutrition related field (MPH or MS).
ÃÂ¢ÃÂ¢Minimum of two years clinical/community experience to include one year of supervisory experience. Previous experience in WIC preferred.
ÃÂ¢ÃÂ¢Requires current Illinois licensure as a Licensed Dietitian or Illinois licensure by reciprocity within six months of initial date of employment.
ÃÂ¢ÃÂ¢Requires current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) certification by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics.
ÃÂ¢ÃÂ¢Requires Certified Lactation Counselor (CLC) or completion within one year of hire and International Board Certified Lactation Consultant (IBCLC) within two years of hire date.
ÃÂ¢ÃÂ¢Ability to work on evenings, weekends, holidays and overtime if necessary.
