Job Description and Requirements
The Renal Dietitian evaluates, develops and implements nutritional care plans. This position provides nutritional counseling services to renal patients, ensures quality service through the safe and efficient use of resources and coordinates care with nursing home dietitians.
~Completes a monthly Nutrition Assessment on all patients;
~Develops an individual plan of care for each patient with nutritional guidelines;
~Documents and visits all dialysis patients to review aspects of nutrition care at least monthly based on pertinent lab results and changes in physician status;
~Delivers counseling service to patient and family members and obtains feedback; and
~Documents encounters in Electronic Medical Record (EMR).
~Attends team meetings and interdisciplinary review of patients;
~Develops QA/PI nutritional initiatives and participates in regular performance improvement meetings;
~Participates in education for pre-End State Renal Dialysis patients;
~Provides in-services to dialysis team members; and
~Serves as educator for team members.
~Maintains clinical staff competencies; and
~Ensures all required regulatory training attained annually.
~BA/BS in Nutrition or related field of study
~At least 12 months experience as a Registered Dietitian as required by State regulations.
~Experience as a renal dietitian preferred.
