Salary: $25-$26/ hour
Shift: Tuesday-Saturday 8am-4pm

Position Summary
Responsible for the nutritional care of residents in long term care and patients in the rehab area. Serves as nutrition leader on the interdisciplinary team to ensure assessments are done, nutritional needs are met and is responsible for nutrition education of residents, patients and staff. Works with dining leadership to develop menus that meet state and local requirements.

Minimum Qualifications:
Requires bachelors or advanced degree from and is a Registered Dietitian with Academy of Nutrition and Dietetics. Masterâs degree preferred. CSG desirable.

Requires experience as a dietitian working in long term care.
Knowledge, Skills and Abilities:
Requires knowledge of CMS regulation in regard to long term care. Must be able to assess and care for individuals with a wide variety of diagnoses and conditions, including parenteral and enteral nutrition. Requires knowledge of menu planning.

Conducts timely screening and assessment of residents and patients.
Develops appropriate nutrition care plans.
Accurately charts in the EMR using accepted standards of care as related to age and condition.
Initiates communication with other care staff ensure nutritional needs are met.
Acts as liaison between medical and nursing staff and the dining services staff.
Provides clinical expertise and oversight to diet technician.
Provides education for patients, residents, caregivers and dining staff.
Reviews menus as needed.
Assists in quality management data collection and participates in continuous quality improvement.
