Job Description and Requirements

The incumbent is responsible for the management and oversight of community dining sites, and coordination and delivery of nutrition services to include intake, ordering of homebound meals Ãâ frail and non-frail, nutritional assessment, nutritional counseling and education; menu development, analysis and nutrition intake. Incumbent is also responsible for the coordination and implementation of outreach and public relations activities with other community agencies and aging programs.

Major Responsibilities/Activities:
Community Dining Center Operations:
* Oversees food service activities at all Ward 5 Community Dining sites and supervises Community Dining Site Managers and Nutrition Aide
* Monthly or quarterly, reports due to the D.C. Office on Aging including Efficiency Reports, Supplemental, etc.
* Works with the Ward 5 Services team in developing and evaluating health services to meet the nutritional and emotional needs of the elderly
* Assists the Director in the planning and implementation of in-service training Workshops and activities for site staff (paid and volunteer)

Client Services:
* Oversees registration of all Community Dining center participants by completing the DC Office on AgingÃâs client intake form
* Conducts nutritional assessment of program participants
* Develops comprehensive plans of care to include, but not limited to the provision of nutrition and health information, and education; physical fitness tips, and assistance to clients with medical follow up appointments, food shopping and proper meal preparation
* Makes referral to other service providers appropriately
* Works toward the integration and coordination of nutrition services with all other services available to the elderly
* Provides nutrition education and nutrition counseling
* Provides on going one-on-one counseling, advice and guidance for clients who are at-risk for poor nutrition
* Monitors client progress through telephone reassurance and home visits
* Performs other duties as assigned

Minimum Qualifications:
* Must be a Masters degree registered, licensed dietitian, with at least 3 years experience in food service management
* Must have experience in program management and supervision, preferably involving services for older persons
* Must demonstrate ability in organizing, directing, coordinating, and evaluating nutrition activities
* Excellent oral, written and interpersonal communication skills
* Must understand and be sensitive to the needs of older persons
* Must have knowledge of problems and concerns affecting the elderly
* Must possess skills in community organization/outreach activities
* Must have access to automobile five days a week for field visits
* Performs other duties as assigned

Work Environment:
Standard office environment and clientsÃâ homes.

Essential Physical Functions:
Ability to lift 20 pounds and drive; able to sit for extended periods of time

PC/laptop, computer printer, office copier, telephone, facsimile machine, shredder.
