Job Description and Requirements
Community Health Alliance of Pasadena (ChapCare) strives to pay its workforce at the mid-level or 50% of the salary range. For new hires, we offer pay at less than the 50% mark to allow for growth. Provide individualized medical nutrition care and education to patients within a group of Community Health Alliance of Pasadena (ChapCare) with emphasis on diabetes, pre-diabetes, hyperlipidemia, hypertension, childhood obesity, failure to thrive and Obstetric Servicers.
Essential Duties and Responsibilities
â¢ Develops individualized nutrition care plans based on collection of diet history, nutritional assessment, current laboratory results and medical diagnosis of patients upon referral from their primary care provider.
â¢ Provides culturally sensitive, on-going education and instruction to patients, families and support persons to achieve behavior change goals, which lead to better clinical outcomes. Special emphasis given on dietary treatment regimen, individualized exercise and weight management plans to improve health status and/or prevention of chronic illnesses and associated complications. Requires excellent counseling skills and the ability to relate to multi-ethnic communities and varied learning levels.
â¢ Will counsel patients in an individual one-on-one setting as well as in class setting. Is a team player and customer service. Able to build rapport with patients and staff and demonstrate good problem solving skills.
â¢ Serves as both Diabetes Educator and Registered Dietitian Nutritionist within the health care team including teaching of blood glucose monitoring and insulin administration. Acts as a resource to assure continuity of care and comprehensive disease treatment and management.
â¢ Provides dietary assessment and nutrition counseling to high-risk pregnant women referred by their obstetrician under the guidelines of the Comprehensive Perinatal Services Program (CPSP).
â¢ Assures utilization of appropriate community resources in the care of the patient. Assists patients in acquiring services from appropriate community agencies.
â¢ Develops and maintains educational materials that are comprehensive and current in English and Spanish.
â¢ Maintains confidentiality and professionalism when handling patient medical charts and personal information.
â¢ Continues to expand and update knowledge in medical nutrition and improve counseling skills through continuing professional education opportunities.
â¢ Proficient in computer applications: Microsoft Excel, Word, Outlook, Power Point and computer charting programs.
â¢ Registered Dietitian in good standing and credentialed by the Commission on Dietetic Registration.
â¢ Diabetes Educator (formal Certification with the AADE preferred)
â¢ Bilingual in English and Spanish
â¢ Good Oral and written communication skills
