Job Description and Requirements
Coler is an 815 bed, leading nursing facility dedicated to providing quality medical, sub-acute, rehabilitative and long-term specialty services. We are committed to serving New York City, providing services to individuals requiring geriatric, memory care and rehabilitation services.
NYC Health + Hospitals/Coler is looking for a Registered Dietitian who will under administrative direction, organizes, plans and directs the operation of the dietary service.
Examples of Typical Tasks:
Candidate should have strong clinical skills, a proficiency in medical nutrition therapy and a minimum of 1 year in the field of nutrition support. Under the direction of the Clinical Nutrition Manager, the Registered Dietitian will be responsible for collaborating with members of the health care team to provide medical nutrition therapy. Identifying patients at nutrition risk. Assessing nutritional needs and identifying nutrient requirements according to patient age and disease specific needs. Developing and implementing nutrition care plans for assigned patients. Instructing patients in principles of nutrition and modifications of normal diet. Monitoring and evaluating outcomes and effectiveness of medical nutrition therapy interventions. Completing MDS 3.0 assessments as assigned. Participates in clinical team Rounds (enteral and ulcer). And performs related tasks as needed.
For appointment to Assignment Levels III and IV
1. Registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.
Special Note for Dietitians assigned to Post-Acute Care (Long-Term Care) facility:
Under the Medicare and Medicaid Programs, Reform of Requirements for Long-Term Care Facilities, as outlined in Section 483.60 of Title 42 of the Code of Federal Regulations all newly-hired Dietitians, Levels I-IV assigned to a Post-Acute Care (Long-Term Care) facility, are required to have New York State Certification, issued by the New York State Education Department (NYSED), as a Dietitian-Nutritionist at the time of hire. Dietitians, Levels I-IV hired prior to November 28, 2016, assigned to a Post-Acute Care (Long-Term Care) facility are required to obtain New York State Certification, issued by the NYSED, as a Dietitian-Nutritionist on or before November 28, 2021.
2. Certification of CNSC (Certified Nutrition Support Clinician)
