Responsible for implementing the Special Supplemental Nutrition Program for WIC as directed by the Senior Public Health Nutritionist Supervisor.

Assess anthropometric, medical, and dietary data to determine clientâs eligibility for the WIC program in accordance with policies of the State WIC program and regulations stated in DHM-150-24. Provides nutrition counseling to WIC participants and promotes client centered nutrition goal setting. Document services in FL WISE using SOAP charting method. Develop, monitor, evaluate and revise nutrition care plans for individual clients at high nutritional risk. Monitor and follow protocols for nutrition assessment and follow up for high risk WIC participants. Develops care plans and follows up with medically high risk clients in providing medical nutrition therapy counseling according to guidelines set up by Academy of Nutrition and Dietetics and as stated in DHM-150-24. Discusses clientâs progress with other professionals. Assess medical documentation forms to determine need for special formula or non-contract formula following the guidelines stated in DHM-150-24.

Provide referral to other health care and social service programs. Coordinate services with other CHD programs, including immunizations, primary care, Healthy Start, etc.

Subject to emergency duty in the event of a natural disaster

Completion of all mandatory or assigned trainings within the required time frame

Participate in breastfeeding promotion (one-on-one or outreach), breastfeeding training and activities (including World Breastfeeding Week), and events as needed.

Has computer access to HMS. Adheres to Confidentiality and Security policies and procedures.

Must have a valid Florida Driverâs license.

Responsible for securing the designated information set for protecting confidentiality, data integrity, and appropriate access, for all information, both confidential and public record, which is stored in hard copy or electronic formats.

Accurately completes all required forms and reports including but not limited to Employee Activity Report (EAR), Missing Bloodwork, High Risk Contacts, Leave and Attendance Records, and travel logs by required deadlines.

Participation in the preparation and selection of information materials and presentations, including secondary nutrition education resources, bulletin boards, handouts, employee wellness, etc. for use in the WIC nutrition program and for various community groups. Participate in WIC and other surveys as required.

Perform anthropometric and hemoglobin measurements for clientsâ certification, recertification, and follow-ups as needed.
Valid Licensed Dietitian/Nutritionist in the state of Florida.
