DESCRIPTION:

This is specialized work under the direction of a nutritionist in a WIC unit. It requires a high degree of accuracy, involves close interaction with other personnel of the CHD, and maintenance of confidentiality. Travel to all sites may be involved in the performance of duties.



REQUIREMENTS:

Bachelors or master's degree in nutritional science, community nutrition, clinical nutrition, dietetics, or public health nutrition. Licensed as a Dietitian/Nutritionist or be eligible to practice dietetics. MINIMUM QUALIFICATIONS PREFERRED: Bi-Lingual â English/Spanish.

EMERGENCY DUTY: Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or perform other emergency duties including, but not limited to responses to or threats involving any disaster or threat thereof, man-made or natural.



MAJOR DUTIES AND RESPONSIBLITIES:

NUTRITION:
Determines nutrition risk eligibility of clients.
Provides individual nutrition education contacts, adapts nutrition education content based on need of the client using the VENA concept to provide appropriate nutritional counseling. Documents services provided
Correctly identifies all high-risk clients and medically high-risk clients, completing an individual high-risk care plan using the SOAP format.
As necessary, refers client to appropriate outside agency, documenting referral. Assigns appropriate food packages. Completes FL-WiSE System data entry ensuring to complete all required screens before the end of the day services are provided


RYAN WHITE SERVICES:

Plans, implements and coordinates the nutritional care component for clients in the Ryan White clinic.
Provides nutrition assessment and counseling as part of regular clinic services.
Develops individualized plan of care and provides follow-up, case management and referrals as needed.
Accurately documents required client information into Care Coordination Module in HMS in
a timely manner.
Evaluates the effectiveness of nutritional care and develops strategies to improve care.


ADMINISTRATIVE:

Acts as a Property Custodian Delegate overseeing the tagging, controlling, transferring, and disposal of the
state's tangible personal assets in the WIC program.
Assists with the annual physical inventory and designates an inventory custodian to conduct the inventory.
Completes and submits the annual inventory report.
Reviews end-of-the month inventories submissions for special formula, ID cards and supply orders.
Consolidates site inventories for EBT Cards, special formula and breastfeeding inventories for all WIC sites into respective master inventory templates monthly.
Creates new site templates post reconciliation monthly.
Assists with inputting monthly appointment schedule as per direction of site supervisor.
Provides professional and administrative support to site supervisor.
Keeps site supervisor informed of problems that may arise and assist with possible solutions.
Completes assigned administrative projects and reports in a timely fashion.
Notifies site supervisor and WIC Coordinator of problematic trends and occurrences.
Assists with maintaining WIC administrative records per policy and regulation.
Meets deadlines for submission of EARs, timesheets, mileage voucher and other requested items.
Performs related duties as required by Supervisor.
Participates in quality assurance activities
Serves as a backup for ordering, receiving, and submitting required documentation to Central Office of special formula.


BREASTFEEDING:

Protects, promotes and supports breastfeeding; assures local site reflects a Breastfeeding Friendly
atmosphere.
Assists with organization and maintenance of breastfeeding supplies for site and issues manual breast pumps.
Promotes breastfeeding classes with all prenatal clients.
Refers clients to Breastfeeding team as needed.
HEALTH TECH:

Collects and records medical data (height/length, weight and hemoglobin levels) of WIC clients.
Transcribes all medical data to the FL-WiSE system.
Completes the Lab/Anthro screen with proper measurement conversions for women, infants and children.
Properly maintains bio-hazardous materials.
Follows FDOH and Hendry CHD policies regarding prevention of blood borne pathogen transmission.


OTHER:

Participates in on-going nutrition education activities with local agency.
Provides nutrition education and general services at Outreach events.
Performs other duties as required.
