Duties and Responsibilities:
The incumbent is part of a global public health organization and is expected to support the Florida Department of Health in Polk County in its mission to protect, promote, and improve the health of all people in Florida through integrated state, county and community efforts and its vision to be the Healthiest State in the Nation. NUTRITION EDUCATION & COUNSELING
This position serves as the nutritionist for the Ryan White Clients served by the Specialty Care Clinic in Bartow, but covers all of our patients in Polk, Highlands, and Hardee Counties. Duties include but are not limited to:
-Completes the medical history, nutrition history, and related assessments for RYAN WHITE clients
-Develops, monitors, evaluates, and revises nutrition care plans for high risk, medically high risk and low risk RYAN WHITE clients and provides/documents nutrition and dietary counseling according to all local, state and federal guidelines Assigns/updates client food recommendations according to program guidelines *
-Provides nutrition programs and outreach to health care professionals and members of the community as appropriate
-Completes inventories as assigned
-Provides classes and group nutrition education for Ryan White clients and their families
Provides appropriate medical nutrition therapy based on the results of the diet history, diet recall, impedance testing, laboratory values, psycho-social factors, and additional medical information from providers.
Documents client counseling and all other contacts in the medical record. Individualizes the Care Plan for each client, including goals and a plan for follow-up.
Conducts nutrition in-service programs for clients, staff, and the general public.
Prepares HIV program specific client education literature.
Maintains demographic, health, clinic, and outcome date for HIV grants. Maintains other statistics as necessary.
Completes records and reports for any program area of the grant(s), meeting ail deadlines.
Attends meetings and in-service programs as necessary to stay abreast of changes in the HIV field.
Performance other assigned duties as requested by supervisor.
Knowledge, skills and abilities, including utilization of equipment, required for the position:
-Knowledge of nursing principles and techniques
-Knowledge of human anatomy and physiology
-Skill in the use of medical equipment and instruments used in providing nursing care
-Ability to demonstrate manual dexterity
-Ability to work with patients
-Ability to interpret and follow medical instructions
-Ability to observe patients symptoms and reactions to treatment
-Ability to communicate effectively
-Ability to establish and maintain effective working relationships with others.
Licensure/registration/certification requirement (If applicable, list the appropriate Florida Statute or federal regulation cite):
-Bachelors degree (or a Masters degree or higher) from an accredited college or university with the major course of study in public health nutrition, nutrition, dietetics, food and nutrition, or food service management.
-Licensure as a dietitian/nutritionist with the State of Florida
