Job Description and Requirements
Location: 2400 Belvidere Road, Waukegan IL
Hours: Monday - Friday (Hours will be determined)
Under the direct supervision of the Senior Dietitian, provides medical nutrition therapy, nutrition counseling, community outreach/education, appropriate referral and follow up for clients and families based on identified high-risk condition and individual needs. Work involves direct client contact in a clinic setting and is performed in accordance with state and local agency policies and procedures.
â¢Assesses and evaluates nutritional needs of clients with high-risk conditions referred by the Special Supplement Nutrition Program Women Infant and Children (W.I.C.) Certifying Health Professional and/or Primary Care Medical Provider.
â¢Provides medical nutrition therapy including development of an individual care plan, nutrition counseling using motivational interviewing, breastfeeding support and follow up for clients and families based on assessed nutritional risk factors and individual need.
â¢Provides WIC certification for eligible clients based upon evaluation of dietary, anthropometrics, clinical and health history data in accordance with established policies and procedures.
â¢Demonstrates knowledge of sound nutritional practices for normal nutrition, as well as appropriate medical nutrition therapy for individuals with a diagnosed medical condition.
â¢Provides consultation services to WIC and Primary Care medical staff as needed to assist with ongoing plans of care for nutrition management of medical conditions.
â¢Demonstrates age specific competency in the provision of services to clients.
â¢Refers clients/families to Family Case Management and other health and social services agencies as needed and verifies that high-risk clients obtain services from referrals.
â¢Documents clinical information including nutrition care plans in clientsâ electronic health record (NextGen and Cornerstone) according to agency and state standards.
â¢Participates in gathering and maintaining programmatic /outcome data and completes required forms/reports (including timecard) in a timely manner.
â¢Participates in the integration of Nutrition Services with other Lake County Health Department and Community Health Center (LCHD/CHC) programs including Prenatal, Pediatrics, Dental, Family Case Management, Early Intervention and Immunizations.
â¢Participates in Continuous Quality Improvement activities to assess and monitor program services and develop plans for improvement as needed.
â¢Develops and presents nutrition education/in-service training for clients/patients, clinic staff and the community.
â¢Issues Medically Prescribed Formula in compliance with state and local agency policies and procedures.
â¢Assists in maintaining office, clinic security by locking up equipment, WIC Food Instruments, Farmersâ Market coupons and other supplies.
â¢Acts as an alternate for other sites as required.
â¢Assists in the orientation and training of new staff and dietetic interns.
â¢Assists in developing and reviewing nutrition education materials for health promotion.
â¢Participates in mandatory in-services and drills, attends staff trainings and other continuing education appropriate for professional development and job duties.
â¢Assists and encourages clients in identifying self-management goals.
â¢Demonstrates adherence to National Patient Safety Goals.
â¢Develops and maintains a basic knowledge of services available at the LCHD/CHC.
â¢Develops and maintains a basic awareness of the Chronic Care Model Processes in order to reduce health disparities within our communities.
â¢Develops and maintains a working knowledge of individual Emergency Preparedness roles.
â¢Reports all unusual occurrences that may involve the safety and well being of the clients and co-workers, directly to supervisor and/or Safety Officer.
â¢Performs duties in a prioritized, organized, and orderly manner to maximize clinic efficiency and productivity.
â¢Maintains orderliness and cleanliness of clinic and work area.
â¢ï· Requires a Bachelorâs Degree in Nutrition related field.
â¢ï· One-year experience in clinical/community nutrition experience desired.
â¢ï· Requires current Illinois licensure as a Licensed Dietitian or Illinois licensure by reciprocity within six months of initial date of employment.
â¢ï· Requires current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) certification by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics.
â¢ï· Ability to work on evenings, weekends, holidays and overtime if necessary.
