Clinical Dietitian / Nutritionist

We provide medical nutrition therapy counseling to patients with diabetes and chronic kidney disease in the comfort of their own home.
Our referrals come from general practitioners, home care doctors, home health nurses, home health agencies, diabetes educators, hospital discharge planners and many more groups. We have full time sales managers marketing dietitian services throughout Chicago and the surrounding suburbs. Our business is growing and we are seeking dietitians who have an entrepreneurial spirit!
â¢ The registered dietitian documents patient visits for medical nutrition therapy using the Nutrition Care Process (NCP) in accordance to the specific plan of treatment and plan of care
â¢ Communicates nutrition plan of care to referring physicians, home health nursing agencies and other providers as needed
â¢ Manages assigned patient population; including retaining patients while maintaining patient-practitioner communications on a regular basis
â¢ Counsels and provides motivational interviewing strategies on various nutrition diagnoses and interventions
â¢ Acquire finger-stick A1c and Lipid panel using Point of Care testing
â¢ Record and report outcome tracking data so that it can be interpreted according to care plan to drive results for the companyâs program management
*This position is an excellent opportunity to obtain your professional practice experience for becoming a Certified Diabetes Educator (CDE)
â¢ Registered Dietitian Nutritionist (RDN) by the Commission on Dietetic Registration
â¢ Licensed dietitian/nutritionist (LDN) by the Illinois Board of Dietetics and Nutrition
â¢ Excellent patient service and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions
â¢ Comfortably provide clinical care and excellent judgement to patients using current practices and guidelines
â¢ Self-directed and able to work with little supervision, has good organizational skills
â¢ Is computer literate and willing to embrace company technology -- technology that eases documentation workload and supports team communication

â¢ Bachelorâs degree
â¢ Accredited, supervised practice program completed
â¢ Registered Dietitian: 1 year
â¢ Licensed Dietitian: 1 year

â¢ Illinois Dietitian License
â¢ Dietitian Registration
â¢ Valid Driverâs License
